PREPARE AT YOUR OWN PACE
Need another set of eyes?
Turn an uncertain situation into a clear question, a record list and a next step. You do not have to accuse anyone or send anything to DRTE.
Your notes stay in this page. This tool does not upload your answers, use AI, save to browser storage or send email. Download or print before leaving. Closing or refreshing may lose your work; your browser may also restore form entries. This is not an encrypted vault or an anonymous channel.
Use a device and browser you trust. Employers, shared devices, extensions, cloud sync and downloaded-file backups may expose activity. The hosting provider can process connection logs. Use roles or initials rather than sensitive identifiers.
For a paper-only version, download the blank worksheet. All entries are optional. Nothing here is reviewed by a person unless you later choose to share it.
Before you share anything
- Preserve the received original and work on copies. Keep source, dates, attachments and a handling log. Do not remove metadata from preserved originals; review metadata and permanent redaction on separate sharing copies.
- Access only material you are authorized to obtain. Do not bypass security, impersonate someone, use tracking links or collect private information simply because it is accessible. See 18 U.S.C. § 1030.
- Public-record access does not authorize every use or disclosure. Check applicable restrictions, court orders and confidentiality obligations. See Arizona’s public-record inspection statute and our recording-law directory.
- If records may be relevant to a legal matter, seek advice about preservation duties before deleting or changing them. Forensic collection may require a specialist. NIST preservation guidance explains the professional considerations.
- Ask a qualified lawyer promptly about retaliation, deadlines, privileged documents, employment restrictions or personal safety. Using this workspace does not file a complaint or pause any deadline. OSHA covers specified laws, not every reporting situation: read its program FAQ. Find legal help through the State Bar of Arizona’s public resources.
Two ways forward: if you want help understanding a record, use Ask a Question with a short nonsensitive summary. If you are considering reporting wrongdoing, consider independent legal or professional advice first, then read DRTE’s intake information before choosing whether to contact us.
No anonymity, attorney-client privilege, confidential source protection, legal outcome or response time is promised by this page. Do not send a sensitive file until the recipient, access, retention and transfer arrangements have been clarified. This is general research education, not legal advice or an emergency service.
